Number Porting Request

I hereby declare responsibly that:

1. The details stated in this request are true and accurate.
2. I am a subscriber on the Donor Provider's network and user of the telephone connection(s) or service(s) listed above.
3. There is no other pending porting request for the specific number(s).

I hereby give explicit instruction to the Donor Provider to disconnect the number(s) listed in this request from its network, and to execute the requested porting of the number(s) to the Recipient Provider's network.

I hereby terminate the connection agreement(s) with the Donor Provider for the number(s) whose porting I request, fully accepting responsibility for fulfilling any obligation I may have toward the Donor Provider.
